Doula Working Service Agreement *

I understand that the role of a doula is to provide emotional, physical, and informational support throughout the process of labor and birth. In no way is this support to be considered medical advice and in no way is Fullbirth Inc. or its employee, Bonu deCaires, of 59 Walnut Street, Montclair, NJ 07042 liable for the outcome or process of the labor and birth. I agree to hold Fullbirth Inc. and its employees or subcontractors harmless for any possible birth outcome. As your doula, Ms. deCaires will meet with you in your home to discuss your ideas and concerns about the birth experience, provide needed information and education and discuss the role of herself and other people to be present at the birth. She will be available by phone as needed or desired while she is on call for your birth. During early labor she will be in phone contact, offer suggestions and answer any questions you may have. When labor enters the active phase or when you feel that her presence is necessary she will come to your home, birth center or the hospital and remain with you for one to two hours after the birth. Ms. deCaires will not perform clinical tasks, such as blood pressure checks or vaginal exams. Her main role is to provide physical comfort, emotional support, information and advocacy. She will not make any medical decisions for you, but will provide needed information and discuss options in order for you to make the best decision for yourself. She will aid your interaction with your caregivers but you or your partner will speak to them on your behalf. After the birth Ms. deCaires will continue to be available by phone for support and questions. She may refer you to a lactation consultant or your pediatrician if needed. She will provide a follow up visit in your home to see how you are doing, review the birth, make sure you are receiving the care you need and to admire you and your baby. If there is no labor and you give birth by scheduled cesarean then support will be provided in the recovery room after the birth and in the postpartum room once you are moved. This support will focus on feeding the baby, facilitating bonding and skin to skin contact, and helping you with recovery. In addition, two postpartum visits will be provided in your home rather than one. If you decide not to call Ms. deCaires while in labor the fee is not refunded. If you have a precipitous labor where Ms. deCaires is unable to reach you in time for the birth the fee is not refunded. The fee for Fullbirth doula services is $3,600. It includes the prenatal visit, phone consultations, being on call 24 hours a day from your 38th week, providing a back-up doula in case of Ms. deCaires unlikely inability to attend the birth, labor support and the postpartum visit(s). An initial non-refundable retainer of $1800.00 is required. The balance is due at the prenatal visit or 36 weeks whichever comes first.
